STATE REQUIREMENTS PER GRADE

The following grades have state requirements. Dental and vision screen forms can be found under the Forms section of the Health heading (click here to be directed to that page) or the nurse can provide you with a copy. Lead screening is usually done by your child's doctor at a yearly wellness check, ask for a copy or that the results to be faxed to the school. Immunizations besides the meningitis vaccine must be up to date within 60 days of starting school. Grades requiring the meningitis vaccine need to have that completed prior to the first day of school. If you have any questions about these requirements please see the school nurse.

Hearing Screens:

NWAEA staff completes hearing screenings annually for students in preschool through 5th grade and 8th grade. Also, if you are new to the district in other grades they will screen your child as well. Parents will be notified only if there is a concern regarding the results of the screenings. Please notify the school nurse, or secretary, in writing if you do not want your child to participate in the annual screenings.
